Visiting the Orthotic and Prosthetic Centers

There are a few things you can do to make sure you have the most positive and productive first vist.

Here’s what to do:

1.

Schedule An Appointment.

You can request an appointment online via email or by calling an office convenient to your location.

2.

Arrive 15 Minutes Early.

Please arrive 15 minutes before your scheduled appointment to complete our Patient Registration documents. These documents are available below for your convenience.

3.

Bring your insurance
information & prescription.

IMPORTANT, please bring your insurance information & the prescription from your physician with you to your first appointment.

Your First Visit

When you arrive

Please have a current copy of your insurance information with you. We’ll ask you to fill out some forms while we make a copy of your insurance or Medicaid/Medicare information. We recommend arriving a few minutes early, so you have some extra time to fill out the necessary information.

Your initial evaluation

During the first visit, your practitioner will evaluate your condition and ask you some questions about your need for an orthotic or prosthetic device. The more information you can give your practitioner the better we will be able to meet your personal needs.

Measuring & Casting

Following the evaluation, the practitioner may measure and/or cast you for your device. Measurement can be taken with different devices such as a tape measure, M-L gauge, calipers, etc. Casting your limb or torso can be done using thin fiberglass material or plaster. Both are removed when dry and easily cleaned from skin with soap and water. After measuring and/or casting is completed, then you will be instructed to make a return appointment for your fitting before leaving our office.

Patient Registration Documents

Please download, printout and fill-out these documents. Bring them with you to your first appointment.

Patient Satisfaction Survey

Your Opinion Matters!

After your appointment, you’ll be emailed or mailed a satisfaction survey with your “personal survey code”.

Please fill out a survey to let us know how satisfied you were with your visit.

How the results are used.

Knowing how our patient’s feel about the level of care we provide is critical. This survey allows us to provide better care for our patients. It helps us…

• Understand what’s important to patients

• Become more responsive to patient needs

• Identify issues early and take action

• Improve patient outcomes

• Meet professional accreditation standards and requirements

By providing direct feedback through these surveys, you can better inform your clinicians about your current situation and how they can improve the care process for you and other patients.

In addition, the data helps us to quantify how specific treatment protocols impact overall patient outcomes. Your input could have a dramatic impact on how the care process is improved for countless other patients such as yourself.

We take feedback seriously.

Surveycare.com is the HIPPAA compliant portal created specifically for orthotic and prosthetic patient feedback. The entire process is secure, fully HIPPAA compliant, and treats all patient information with utmost confidentiality.

To fill out your survey “online”, click on the button below to be directed to surveycare.com.

Enter your “personal security code” to launch the satisfaction survey.
